25.01.2022 I’d just finished competing at the Ironman World Championships. I was the fittest I’d ever been in my life. I’m talking one of those lean and toned, honey-limb...ed, sunglass-wearing triathlon types. It was a Friday morning and I was smugly (yes, smugly) working out at my local gym. I’d just finished up on the treadmill and had hopped onto the rowing machine, Biggie Smalls blasting in my headphones. You know those wankers that can’t keep their eyes off themselves in the gym mirrors? Yeah, that was me I started building up momentum on the rower, sweating vigorously, bopping along to the dulcet tunes, one eye trained on my reflection, my mind lost in the ego-mania of my workout daydreams. I was Lara Croft. That guy Liam Neeson plays in Taken. A messenger for the Queen. I was rowing for my family, my country, the boys back home!!! And, then, all of a sudden, I wasn’t rowing at all. I’d lost my balance and, with my feet still firmly clipped into the plastic feet of the rowing machine, I fell out of my seat, sprawled out across the floor in a sweaty pool of shame. I’ll be clear, this is not my most embarrassing life moment. I’ve gone to the bathroom at Westfield and walked around for a good hour with my skirt tucked into my underwear before a motherly type came up to me and let me know of my predicament. I’ve dropped in on an extremely talented (and v. cute) surfer and ruined his board. I’ve accidentally parked my car in a bus lane and endured an extremely loud telling off by an irate bus driver. I’ve slipped over, had green spinach stuck in my teeth, farted in front of a group of yoga participants and a mere two weeks ago, I picked up some takeaway from a local restaurant and walked face first, with speed, into the glass wall of the dining area. Which I thought was the exit. All of this to say: You can be nailing your studies and still fail an exam. You can win a business award and lose money on a product launch. Or you can compete at the epitome of endurance events and still fall out of the rowing machine at your local gym. Mistakes happen. Life is clumsy. 24.01.2022 Lips This beautiful patient required balance and volume for her lips. I have used dermal fillers to re shape and add volume. The result is a beautiful soft even pout. As I always say its not just about filling , we need to consider the face as a whole and restore balance. This does not mean we have to create perfect symmetry in all cases but enhancing the overall look for a patient is the goal. I just love the power of dermal fillers. 23.01.2022 I cant wait until I get back to work to do my yearly IPL sessions @drguywatts clinic. Over the summer I tend to get more freckles and solar lentigines. This summer add on being pregnant and I have developed slightly more pigmentation. I will show you the process and before and afters images of my own skin in a few months when we are all up and running again. IPL skin rejuvenation works best on the more fair skin types. 19.01.2022 Lip filler blindness is a thing!!! and I see it happening more and more. Not actual blindness, but patients becoming comfortable with a new augmented norm. They are blind to how disproportionate their lips look in relation to the rest of their facial features. The story usually starts with a little lip filler. They may feel its not quite right and feel they want a top up. This happens, and then they might think 6 months later the product has gone or completely broken down. T...hey return to their injector for more. In reality there is still product left in the lip, their expectations for what they want may never be achievable but some injectors keep filling and filling. The injectors may try to chase tiny imperfections and asymmetries. In reality it is extremely rare to find someone with perfectly symmetrical lips. It is ok and normal to have slight asymmetries. When lips are filled and filled, the product can slightly migrate upwards. Cue the duck lip. The filler can move just above the lip border and thats the look we cant help but stare at, the rubbery fish lip/duck lip. Its just not pretty. Bad technique can also lead to this problem despite volume administered. When dermal filler is administered correctly, you can still achieve plump volumous lips that suit your face as a whole. . As a registered nurse my number one priority is to make sure patients have not lost the insight to objectively look at their faces. The nursing code of conduct and standards of practice that we follow ensure we are patient advocates first and foremost. Safe and ethical practice is a priority. Secondary to this, the artistic flare to understand faces and know what will make someone look more beautiful, not just different is certainly a huge part of the aesthetic industry. 13.01.2022 LETS JUST SAY...A complete understanding of the anatomical locations of vasculature structures, as well as how facial aging or previous surgical alterations may change the structural orientation, will aid the clinician in identifying critical danger zones to avoid during injection of dermal filler. Descending and thinning skin may make it easier to see these important structures. Surgeries can impact the location of these anatomical danger zones as well. It is very import...ant to take note of prior facial surgeries and the history and location of other soft tissue fillers and volume enhancers during the initial aesthetic consultation. Be sure your injector is aware of danger zones but more importantly if they know what to do in the case of adverse events. . . Actions that decrease the potential complications of dermal fillers are: Aspirate before injecting Inject in a retrograde fashion Inject small aliquots of filler Avoid using anesthesia near a vascular bundle that may induce vascular spasm, such as those containing epinephrine; also avoid using epi- nephrine so that the cause of blanching can be determined quickly Use the smallest gauge needle possible to slow the flow of product Pinch/tent the skin to provide more space be-tween superficial branches of main arteries and to move away from underlying vasculature Use a reversible product . . 12.01.2022 We are currently in social isolation not lockdown. What this means is: 1. We should NOT be socialising with family, friends or colleagues that live in another house other than via video link etc! 2. We should NOT be gathering in groups. 3. We should NOT be replacing a night out with a night in and inviting all our friends round. 4. We should NOT be letting teenagers and young adults gather in groups around parks and public places... However, as we are NOT in total lockdown you CAN continue to: 1. Go for a walk with your own household (but staying a good distance from others eg 2-3 metres) 2. Drive to national parks, nature reserves etc (some have removed parking charges) and walk in the vast area away from others 3. Walk your dog 4. Head out to a shop for essentials, prescriptions etc for yourself or someone who is vulnerable and isolating, then head home. IF people continue to socialise in a normal way, a full lockdown will likely be enforced on us to protect the health of our most vulnerable. PLEASE DO NOT judge people for taking a walk in the fresh air and sunshine; but equally PLEASE DO NOT carry on as normal. PLEASE keep your distance from others and do not arrange social gatherings. Make the most of getting out for a walk with your own household and catching up on jobs or leisure activities at home. 11.01.2022 Dermal fillers should be undetectable! When administered correctly a more youthful, volumised or flawless look is achieved. The first thing a new patient will say is.. I want to look natural, I only want a little bit and I dont want people to know Of course I do have patients that say fill me up and freeze me ahaha but not often. My approach is lifting and framing the face. Sagging is a common complaint. A patient may feel they look tired, sad or that everything is heading... south. Dermal fillers can be administered to improve the look of sagging. I often will use 2-3mls for a patient that will be strategically placed in several areas of the face to achieve the lifted look and be very undetectable, 3mls spread over the face IS NOT A LOT!. 10.01.2022 Often people may comment why are you doing all of that to your skin? You dont need to do anything to your skin! (Thanks Dad) haha. The reason why people have healthy glowing skin is because they put the effort into regular skin treatments over time and are using a good skincare regime at home. You will reap the benefits of looking after your skin now, well into your later years. Same goes for if you never wore sun protection growing up, and suddenly now you are noticing... pigmentation, lines and wrinkles. It is all cumulative. If you can commit to at least a skin treatment every 3 months and good skincare that is a great start. The key is to have a thorough consultation to find out the best treatment for your individual concerns and have a plan put in place so theres no guess work. We all can get swayed by pretty packaging and nice smells but its actually down to your perfect prescription of active ingredients and medical grade treatments that will benefit you most in years to come. Dont just do something to look good for now, you want to look great long term. This lovely lady had 2mls of dermal filler administered into the lips and the marionette lines (corners of the mouth). The key to addressing the lips is to create a beautiful soft pout that still has curves. In this case the upper lip needed more projection and the lower lip needed support laterally. Careful ...placement of the dermal filler has lifted the corners of the mouth and balanced the lips to give a more happy appearance to the whole mouth area. 03.01.2022 Tear Trough Fillers This gorgeous patient was concerned with her under eye area. Many of my patients have this same concern. The reasons may be genetic or early signs of ageing. What you need to know about fillers administered to the tear trough:... They are perfect for filling the sunken area under the eye If you have dark circles due to shadows cast from the sunken area, you will get an improvement in your dark circles. If you have dark cirlcles due to pigmentation, it will not reduce this. Fillers can hide eye or blend bulging eye bags on the lower lid They DO NOT remove eye bags They are best suited to patients who have good skin quality. So if your skin is lax and crepey, you may not get a desirable result. More likely to see lumps and bumps. Crepey skin plus eye bags is best suited to surgery (Blepheroplasty) Note: Something I have become aware of while treating the tear troughs over many years, is that they can last much longer than we once thought on some patients. This is great, but can also have some undesirable effects. The under eye area can become puffy a year or so after the filler has been administered and tends to retain water. If you are one of the unlucky ones that this happens to, I recommend getting the filler dissolved.
