17.01.2022 As a follow up to the practice of performing portable chest x rays through closed doors with windows. Following the previous article featuring the practice as p...erformed in Australia, I thought it might be of value to share what we've been doing at The University of Washington and Harborview Medical Center, along with several other facilities from what I'm reading. In going through the comments to the previous article, it is clear there are still many questions on the specific methods utilized in this practice. If done properly, the images are of EXCELLENT diagnostic quality. I hope my sharing this 'again' might be of help to many. Given ongoing challenges in some regions of the country in respect to availability of PPE for health care providers, along with the continued explosion of cases nationwide, I believe that sharing some unconventional (though resourceful and helpful) tools and techniques may be in order. In many COVID-19 portable chest x ray cases it is possible to save a set of PPE and avoid even entering some patient isolation rooms to perform that portable chest x ray! They can be exposed through the window of a closed door- from OUTSIDE the patients room. Ideally, the image cassette is placed- with our guidance, by a provider already donned inside the room, thus saving a set of PPE and considerable time, then, with our direction from outside- the room lights are dimmed for collimation. Given the significant increase in SID- the "inverse square law" must be employed for exposure calculation- roughly 3-4x the normal chest x ray mAs. This is only possible on at least semi upright patients which are in stretchers or wheelchairs which can be faced towards the windows. Important points for success here: Patients must be semi upright or fully upright. Ideally the tube should be as close to perpendicular to the plane of glass as possible. Exposures typically with our systems- normally, for an average patients were 110 @ 2. With our distances almost doubled, they are now 110 @ 8 or more. Motion can be an issue, so sometimes it may be worthwhile to bump the kVp up to 120 to bring mAs down a bit. It goes without saying that this technique isn’t always possible and not suitable for supine patients or rooms in which the patient stretcher can not be moved so that the patient is facing the door. Though I am likely writing a paper for journal publication, the timeline is not designed to help those currently on the front lines. Much like the guidelines for dealing with a regular pandemic (if there is such a thing) address this particular virus- they do not respond fast enough and protocols have sometimes been a bit behind the curve. The quality of the x rays are superb!!! Even when exposed through double paned windows housing non retractable metal or plastic blinds! ZERO artifacts! Right now, I just want word to get out to those struggling with insufficient supplies of PPE, or tiny exam rooms for which the portable machine might barely fit, and high volumes of isolation patients. If this might save more staff from virus exposure while entering rooms, conserve cleaning supplies since we are avoiding bringing the entire portable x ray machine into that droplet/contact or airborne room, and another set of PPE- then this technique and post could be of tremendous value! For the RN or provider in the room, willing to help place the plate, offer them a lead apron ahead of time, or a lead wheeled shield similar to the ones we use in surgery if they are concerned about their exposure to scatter radiation. Distance, distance, distance! Have them stand in the corner. The dose is really minimal and the benefit far outweighs the risk in my humble opinion. Remember though- everything that goes into that room has to be cleaned- which is labor intensive and time consuming. I am sharing this again to this ASRT forum. When I shared it months ago, to other imaging groups, it did not get the widespread attention it really deserves. The ability to shoot through double pane windows with closed metal or plastic blinds is something many would not even consider. Most of our ER rooms at the University of Washington Medical Center are of this type and we are able to produce images with no obvious visible artifacts. The chest radiography below was performed through one of these double pane windows with metal blinds. I hope this helps!
